The easiest way I have fouind to do the whol process is as follows.

Make a Dos bootable floppy
Download, unzip and copy eXtender from the following URL,
http://freedos-32.sourceforge.net/downloads.html

Boot into DOS from the floppy
from the command prompt, type "x", a space and your exe name eg
x my.exe
